Documentation forms for FEAPs/PECs Mastery
For students who have attended EPI 0945 Orientation 

AS EXPLAINED IN ORIENTATION, EPI STUDENTS MUST PROVIDE A COURSE ASSIGNMENT (HARDCOPY/THUMBDRIVE*) WITH AN ATTACHED ACTIVITY/ASSESSMENT COVER SHEET FOR EACH OF THE 39 INDICATORS LISTED ON THE FIELD EXPERIENCE DEMONSTRATION OF FEAPs AND PECs FORM. THESE WILL BE COMPLETED OVER THE DURATION OF THE PROGRAM AND THEN REVIEWED IN THEIR ENTIRITY AT THE EXIT INTERVIEW (WHICH OCCURS AT THE END OF YOUR PROGRAM).

*A thumbdrive is the only option starting with Cohort 52.

The following interactive forms are to be used as needed as you progress through the mastery of your FEAP/PEC indicators.

ACTIVITY/ASSESSMENT COVER SHEET - This form is to accompany each activity/assessment you choose to submit as evidence of demonstration of a FEAP or PEC indicator. Please be sure to fully complete both sections and to attach additional paper as needed to address the responses. This form is not valid without the signature of your classroom instructor at the bottom of the page. One signed cover sheet is required for each of the 39 indicators listed in the Field Experience Demonstration of FEAPs and FECs form.

Course Syllabi -
