All Students are required to submit their Midterm Exam Proctor Information to the HCC Opticianry Program EVERY SEMESTER.

**Only ONE Submission Form is necessary for all your exams. Simply list each exam needed individually in the "Exams Needed" area of the form.

**Submit this information by the 3rd Week of the semester.
**Submitting before the semester begins make management of the information difficult.
**Submitting later than the 3rd Week may cause you to not receive your exams within the required time to complete the exams or for you to not receive your exams at all.

The submission of Midterm Proctor Information is required of:

On-campus Tampa/ Edison Students
Distance Learning Students taking exams off-campus
and
Distance Learning Students taking exams on-campus
