PRIOR TO STARTING THE HCC OPTICIANRY ONLINE ORIENTATION: Submit the HCC Opticianry Students Data Form to the Opticianry Program. This information will be used to create your Opticianry Student File and to provide your contact information to the Opticianry Program for communication purposes.

FILL OUT:

Fill out the CHECKLIST as you proceed through each area of the HCC Opticianry Online Orientation. This checklist is used by the Student to ensure that they have successfully completed all areas of the orientation. Save this document for your records.

Once you complete the HCC Opticianry Online Orientation you must submit the Orientation Completion Form. Upon completion of the HCC Opticianry Online Orientation please fill out, sign and submit the Online OPT Orientation Completion Form. You will then be contacted about your registration options.
