The Student Data Form is presented in two formats (WORD 2007 and PDF). The WORD 2007 format will allow you to type your information directly into the form and allow you to save the filled-out form to your computer. If your software is not compatible with WORD 2007 please download and print the PDF format. The PDF format will not allow you type information directly into the form. Fill the form out by hand.

PRIOR TO STARTING THE HCC OPTICIANRY ONLINE ORIENTATION: Submit the HCC Opticianry Students Data Form to the Opticianry Program to the email address below (faxes are no longer accepted). This information will be used to create your Opticianry Student File and to provide your contact information to the Opticianry Program for communication purposes.

Once you have submitted your HCC Opticianry Program Student Data Form (SDF) you may continue through this online orientation. After your SDF is received you will receive a confirmation email. If you do not receive this confirmation within 5 business days please email the Opticianry Program: hccoptadmissions@hccfl.edu

FILL OUT:

Fill out the CHECKLIST as you proceed through each area of the HCC Opticianry Online Orientation. This checklist is used by the Student to ensure that they have successfully completed all areas of the orientation. Save this document for your records.

Once you complete the HCC Opticianry Online Orientation you must submit the Orientation Completion Form. Upon completion of the HCC Opticianry Online Orientation please fill out, sign and submit the Online OPT Orientation Completion Form. You will then be contacted about your registration options.
