Open Access Plus – In Network Plan (OAP-IN)

  • You do not have deductibles or co-insurance.
  • You do not need referrals but you must choose doctors within the OAP network.
  • You have a $20.00 co-pay for a primary care physician (PCP) visit. 
  • You have a $35.00 co-pay for a CIGNA care designated specialist visit or a $50.00 co-pay for a non-designated CIGNA care specialists visit.

Enhanced High Deductible Health Plan (HDHP) 

 

  • You have deductibles and/or co-insurance for all services. 
  • HCC contributes up to $750.00 per plan year into your HSA or FSA account. 
  • You can visit any provider or medical facility you choose, in or out of network. If you choose to see providers outside of the CIGNA network, you will pay higher out of pocket costs and your care will be covered at the out of network benefits level.
  • Cigna’s generic preventive prescriptions will be covered at 100%.

  • You have a calendar year deductible/out-of-pocket maximum of $1,500 for employee only coverage or $3,000 total for employee plus spouse, children, or family. This means that once the deductible is met you pay nothing toward medical services for the remaining calendar year.
  • HCC contributes up to $750.00 per plan year into your HSA or FSA account which can be used toward your annual deductible.
  • Cigna’s generic preventive prescriptions will be covered at 100%.
