STEP 1.                  Download.  Read  the procedures for the applications and the program information for
                               the Law Enforcement Academy.

STEP 2.                  Download.  Complete the HCC application for admission to program: VOC.APPL.LAWE
                               Bring application fee ($20.00), if applicable, to your FBAT test appointment.

STEP 3.                  Download.  Complete the Florida State Residency affidavit.  Note:  Items 1 thru 10 in
                              the mid-section of the form must be completed.  Example:  ‘Who is the person who
                              claims to have lived in Florida for 12 consecutive months?  If you are that person, then
                              you are the ‘Claimant”.  Claimant and student is often the same person.

STEP 4.                  Download.  Complete the entire application.  (19 pages)  Bring your application, along with the 
      $130.00 fee to your FBAT test appointment.

STEP 5.                  Download.  Choose your date to take the FBAT Exam.

STEP 6.                  Download.  Complete the form, place your selected date at the top of form in field marked 
                               "Test Date."  Fax the form to: 813-253-7973.
